Every player needs one, once per school year. Without it she cannot take the floor.

Fill out the waiver(opens cpybl.com in a new tab)A player without a waiver on file is not eligible to play, and her team's games may be forfeited. One missing form affects the whole team.
Before you submit
- Check the spelling and date of birth
- The league matches waivers to rosters by player last name and date of birth. A typo in either is the usual reason a waiver does not register.
- One per child, once per school year
- If you have more than one daughter playing, each needs her own.
- Do not submit it repeatedly
- If you have already filled one out correctly and she still shows as missing, the roster needs fixing, not another form. Contact the club and we will sort it.
The waiver is a CPYBL form and is submitted to the league, not to the club. CPYBL's own waiver page has the full detail.
